Referral History (New Clarity Interface)

The History section of a referral displays events that have occurred for a referral, allowing users to see the lifecycle of a referral from creation to completion.

Overview

The History section of any referral includes a complete history of all actions taken for the referral. Users can see a variety of events, including the date and time of the event, and the user/agency responsible for the event.

Accessing a Referral's History

When you click on a referral, you see a page with information about the referral.  The History section is at the bottom of the page and shows all of the activities connected with the referral.

To access a referral’s history, a user with the appropriate access can click on referral from any of these locations:

The Referral: Edit referral details page will appear. Scroll down to the bottom of the page to see the History section.

The table in the History section displays the following columns: 

  • Activity
  • Date
  • Days Pending
  • Staff

A column selector is available so that users can customize their view.

The following events are recorded in a referral’s history:

Status changed to “In-Process.”

Status changed to “Denied,” and referral not reassigned to the Community Queue.

Removal date (user entered).

The referral is moved from one Community Queue to another.

"Moved from Community Queue: <Old Community Queue Name> to <New Community Queue Name>"

Date the referral was added to the old Community Queue (Date the referral was moved to the new Community Queue)

Client connected to program with a Referral Connection.

“Enrolled: <program_name>, <agency_name>”

Enrollment date (user entered).

The Check-in button is clicked for the referral.

“Check-In”

Check-in date.

Navigator assigned using the ASSIGN NAVIGATOR button.

“Assigned Navigator: Staff”

Date assigned.

The same assessment used to refer a client to the Community Queue is completed, resulting in a new score (Use Most Recent Assessment must be toggled on in REFERRAL SETTINGS).

“Assessment score updated.”

Date assessment completed.

The value for Reason for Removal for a referral from the Community Queue is changed.

“Edit Queue Removed attributes.”

Date the value was changed.
