[CLNT-128] Client Enrollment Details (New Clarity Interface)
This article describes how to run the [CLNT-128] Client Enrollment Details report, which provides information on a client's self-reported history of homelessness, move-in date, and destination at exit.

Purpose
The Client Enrollment Details report provides information on client responses to questions about their history of homelessness, as well as move-in and destination-at-exit information. These details can help determine additional periods of homelessness for clients.
Running the Report
Users with the Client Reports access right can run this report. The information returned may be limited by the user's broader access rights and visibility settings.
Report Location
This report is a client-based report and is run from Reports in the client's record.

To run the report:
- Click Reports in the client's record.
- Locate [CLNT-128] Client Enrollment Details in the Available Reports table (use search or sort if needed).
- Click Run on the report's row or select Run from the Action Menu options.
- Enter the report parameters (see Parameters below) and click Submit.
- When the report is ready, open it from the My report queue card or the global Status icon in the header.

Parameters
The following parameters are required to run this report:

Report Details
The report is organized into the following sections: a Header, Enrollment Details, Self-Reported History of Homelessness, and Housing Information.

Report Section: Header
The header section displays the report title (Client Timeline Enrollments), Agency, Unique ID, Client Name, and Date Range (reporting period).
- Date Range (Reporting period): These are the dates selected by the user in the report parameters. Clients are counted if they start, exit, or have an enrollment that overlaps the selected date range.
Report Section: Enrollment Details
The Enrollment Details section displays the following fields for each enrollment listed: Agency Name, Program Name, Program Type, Enrollment Start Date, and Enrollment End Date.
The Agency Name, Program Name, and Program Type match what is currently set up in the HMIS database. The Enrollment Start Date and Enrollment End Date fields reflect the dates entered on the entry screen and exit screen for that enrollment. If the client is still actively enrolled in a program, the Enrollment End Date is blank.
If the client is enrolled during any date range that overlaps with the report period, the enrollment information is shown, even if the Enrollment Start Date or Enrollment End Date falls outside the report period.
Program Type and length of time in each enrollment can be used to identify clients who may meet the definition for chronic homelessness.
Enrollments in Emergency Shelters, Safe Havens, or Street Outreach programs indicate homelessness, but Night-by-Night Emergency Shelters and Street Outreach programs also need appropriate services recorded to document homelessness. The [CLNT-127] Homeless Status Timeline Report shows all valid enrollment and service months to help document chronic homelessness.
Report Section: Self-Reported History of Homelessness
The Self-Reported History of Homelessness section displays the following fields: Type of Residence Prior to Enrollment, Length of Stay in Prior Living Situation, Length of Stay Less than 7 Nights, Length of Stay Less than 90 Days, On the Night Before - Stayed on the Streets, in ES, or SH, Approximate Date Homelessness Started, Number of Times on the Streets, in ES, or Safe Haven, and Total Number of Months Homeless on the Streets, in ES, or Safe Haven.
These fields show information provided by the client at program entry. This information can help determine whether the client appears to meet the definition of chronic homelessness. While HMIS enrollments and services can be used for third-party documentation, this self-reported information from the client, collected at program entry, does not constitute third-party documentation of chronic homelessness. The data above can help fill in the gaps for a client's history of homelessness; third-party verification for institutions or other stays outside HMIS-participating programs may be needed if documentation of chronic homelessness is required.
The Length of Stay Less than 7 Nights and Length of Stay Less than 90 Days fields are used to determine whether a prior living situation of permanent or transitional housing can be considered homelessness:
- For Housing Situations: Did you stay less than 7 nights?
- For Institutional Situations: Did you stay less than 90 days?
If the answer to these questions is Yes, and the answer to "On the night before, did you stay on the streets, in ES, or SH?" is also Yes, the prior living situation can be considered homelessness.
The Number of Times and Total Number of Months Homeless fields can help determine, but not document, chronic homelessness.
Report Section: Housing and Exit Information
The Housing and Exit Information section displays the following fields: Housing Move-In Date, Exit Destination, and Specify Other Exit Destination.
These fields provide information on the client's move-in date within any permanent housing program and show the exit destination entered into the Exit screen of the enrollment. These fields help determine when a client is no longer homeless or begins a break in homelessness.
Note: The Move-in Date will be for project types ( 3, 9, 10, 13). It will also include project type 7 with HUD: Pay for Success funding and project type 6 with VA: Grant Per Diem - Case Management/Housing Retention.
Chronic Homelessness: These reports are tools that can assist in documenting chronic homelessness, but they do not constitute documentation of chronic homelessness in and of themselves. All information on the reports should be reviewed for accuracy and extenuating circumstances.
