Viewing a Referral's History

The HISTORY section of a referral displays events that have occurred for a referral, allowing users to see the lifecycle of a referral from creation to completion.

To access a referral’s history, click the Edit icon for the referral from either of these locations:

The REFERRAL: EDIT page will appear. Scroll down to the bottom of the page to see the HISTORY section.


The following events are recorded in a referral’s history:

Date added to Community Queue.

Created from the REFERRALS tab or reassigned from the Community Queue to a program (but not to a Bed or Unit).

“Program Referral: <program_name>, <agency_name>”

Date reassigned to a program.

Reassigned from the Community Queue to a program and a Unit.

“Program Referral: <unit_name>,  <program_name>, <agency_name>”

Date reassigned to a program.

Reassigned from the Community Queue to a program and a Bed within a Unit.

“Program Referral: <bed _name>,<unit_name>,  <program_name>, <agency_name>”

Date reassigned to a program.

Referred from the Unit Queue.

“Unit Queue Program Referral: <unit_name>,  <program_name>, <agency_name>”

Date referred to the Unit.

Direct referral to a Unit.

Direct Referral: <unit_name>, <program_name>, <agency_name>

Date referred to the Unit.

Direct referral to a Bed.

Direct Referral: <bed_name>, <unit_name>, <program_name>, <agency_name>

Date referred to the Bed.

Status changed to “In-Process.”

“In Process: <program_name>, <agency_name>”

Date status changed.

Status changed back to “Pending.”

“Pending: <program_name>, <agency_name>”

Date status changed.

Status changed to “Denied,” and referral not reassigned to the Community Queue.

“Denied: <program_name>, <agency_name>”

Date status changed.

Status changed to “Denied,” and referral reassigned to the Community Queue.

“Denied: Reassigned to Community Queue: Default”

Date status changed.

Removed from the Community Queue.

“Removed from Community Queue: Reason”

Removal date (user entered).

Status changed to “Expired,” and referral not reassigned to the Community Queue.

“Referral Expired”

Date expired.

Status changed to “Expired,” and referral reassigned to the Community Queue.

“Expired: Reassigned to Community Queue”

Date status changed.

Client connected to program with a Referral Connection.

“Enrolled: <program_name>, <agency_name>”

Enrollment date (user entered).

The CHECK-IN button is clicked for the referral.

“Check-In”

Check-in date.

Navigator assigned using the ASSIGN NAVIGATOR button.

“Assigned Navigator: Staff”

Date assigned.

The same assessment used to refer a client to the Community Queue is completed, resulting in a new score (Use Most Recent Assessment must be toggled on in REFERRAL SETTINGS).

“Assessment score updated.”

Date assessment completed.

The value for Reason for Removal for a referral from the Community Queue is changed.

“Edit Queue Removed attributes.”

Date the value was changed.


Hovering over a referral event will display a tooltip that identifies the item.
