- We have passed the Initial Review of Warning Flags milestone in the FY2025 LSA Season. Data Quality - Missing Elements flags have been impacting many communities. Vendor-level advocacy for additional information, resources, and increased thresholds has continued.

"Missing" is defined to mean data where the related field(s) in HMIS has a value of 99 (i.e., "Data Not Collected"), is null, or is blank.

Please review enrollment data to check if any unknown Disability data can be updated. Disability data is considered unknown when the "Disabling Condition" question has a response of "Client doesn't know," "Client prefers not to answer," or "Data not collected" or when the field is null or blank in HMIS.

DVStatus is missing for more than 10% of adults and Head of Households in this project type and household type. Please review enrollment data to check if any missing DV data can be updated. DV data is considered missing when the "Survivor of Domestic Violence" question has a response of "Data not collected" or is null or blank in HMIS. Per the HMIS Data Standards, Domestic Violence (4.11) is a Common Program Specific Data Element, and not all projects are required to collect this data based on the project's funding source. If this is a contributing factor to the missing percentage in this flag, please reference this in your note.

- Please note, these flags may also appear without these missing elements if enrollments occur outside of Program Operation Dates, HMIS Participation Status Dates, and/or when an ES NbN Enrollment program start/exit date differs from the First Bed Night/Last Bed Night +1 day. This is due to how the LSA specifications adjust start/exit dates. When adjusted start or exit dates do not align with the HMIS start/exit dates, it can cause these flags to return (particularly for LivingSituation, Destination, and Disabling Condition flags).
- In the context of ES NbN enrollments and clients, it is important to recognize that ESTDestination flags may occur in the absence of an Exit Date one day after the last recorded Bed Night. This situation may result in a community surpassing the threshold related to the project and household type. In such cases, the LSA establishes an effective Exit Date with an Unknown (99) Destination.
- Please note, these flags may also appear without these missing elements if enrollments occur outside of Program Operation Dates, HMIS Participation Status Dates, and/or when an ES NbN Enrollment program start/exit date differs from the First Bed Night/Last Bed Night +1 day. This is due to how the LSA specifications adjust start/exit dates. When adjusted start or exit dates do not align with the HMIS start/exit dates, it can cause these flags to return (particularly for LivingSituation, Destination, and Disabling Condition flags).
- We are approaching the December 15, 2025, Initial Review of Warning Flags interim benchmark. By this date, each CoC should have at least one Official HUD Review file successfully uploaded with all applicable notes entered.
- We have received reports from the HDX team that there are no additional, foreseen updates to the Missing Elements thresholds in the new flag series 1351-1360.
November 2025
- HDX 2.0 - LSA Module will open for submissions on 11/10/2025 at 12:00PM ET.
October 2025
- The FY2025 version of the LSA Exports were released to all instances on 10/01/2025.
- The HDX 2.0 Sandbox Environment has opened, and we are actively testing the LSA Export and HDX 2.0 Functionality.
- We anticipate the LSA reporting module in HDX 2.0 to open early to mid-November.
