Report Purpose and Summary

This report is based on Assessments and provides a listing of the assessed scores during the reporting period. The report also includes the responses about Chronic Homelessness, Referral Status, and separate columns for ES EE, ES NbN, TH, PSH, and RRH project enrollments.

Running the Report

Report Location

The report is found in the Administrator section of the Report Library.

Who Can Run the Report

System Administrators, and others with the appropriate access level, can run the report.

Parameters

The following parameters are required to run the report.

CoC Choose which CoC to include in the report
Assessment Processor(s) Choose which Assessments to include
Active on the Community Queue Select Yes, No, or All
Report Date Range Choose the Assessment date range to include
Report Output Format Due to the size of the returns, only Excel is available

Report Details

Universe of Clients

The report includes clients that:

  • Have an assessment from the selected Assessment Processor
  • AND Assessment Date is within the Report Date Range
  • AND Active/Inactive on the Community Queue or All (regardless of Community Queue)

The report includes fields that include:

  • General information about the Assessment:
    • Agency Name
    • Assessment Name
  • Basic demographic information about the client:
    • Unique Identifier
    • Last Name
    • First Name
    • SSN  (last four only)
    • Date of Birth
    • Age (on Assessment Date or Report Start Date, whichever is greater)
    • Gender (Displays picklist value(s) selected in Clarity Human Services)
    • Veteran Status
  • Client Assessment information:
    • Date
    • Score (Headed as VI-SPDAT, but works for non-VI-SPDAT assessments, is displayed if Display Score option is turned on)
    • Disabled
    • Chronic? Calculated field based on the HUD definition
    • Client Entering from the Streets, ES, or Safe Haven
    • Approximate Date Started (Date homelessness started)
    • Continually Homeless for at Least One Year
    • Time Homeless in the Past Three Years
    • Total Months Homeless in the Past Three Years
    • On a regular day, where is it easiest to find you, and what time of day is easiest to do so?
    • Staff Name
  • Client Referral information:
    • Active on the Community Queue
      • Yes - if the client has been referred to the Community Queue under this Assessment and is still pending in the Community Queue
      • No - if the client is not in the Community Queue now or hasn’t been added to the Community Queue under this Assessment
    • Reassigned
      • Yes - if the client has been referred to the Community Queue under this Assessment and then has been reassigned to a program
      • No - if the client hasn’t been added to the Community Queue under this Assessment or hasn’t been reassigned to a program
    • Referral Status - last client referral status at this moment
  • Client Program information:

    • ES EE Program Name - if the client has an ES EE program enrollment (Project Type = 0) during the Report Date Range (the last Program Name if there are several enrollments)
    • ES EE Start Date - if the client has an ES EE program enrollment (Project Type = 0) during the Report Date Range (the last Program Start Date if there are several enrollments)
    • ES NbN Program Name - if the client has an ES NbN program enrollment (Project Type = 1) during the Report Date Range (the last Program Name if there are several enrollments)
    • ES NbN Start Date - if the client has an ES NbN program enrollment (Project Type = 1) during the Report Date Range (the last Program Start Date if there are several enrollments)
    • TH Program Name - if the client has a TS program enrollment (Project Type = 2) during the Report Date Range (the last Program Name if there are several enrollments)
    • TH Start Date - if the client has a TS program enrollment (Project Type = 2) during the Report Date Range (the last Program Start Date if there are several enrollments)
    • PSH Program Name - if the client has a PSH program enrollment (Project Type = 3) during the Report Date Range (the last Program Name if there are several enrollments)
    • PSH Start Date - if the client has a PSH program enrollment (Project Type = 3) during the Report Date Range (the last Program Start Date if there are several enrollments)
    • RRH Program Name - if the client has an RRH program enrollment (Project Type = 13) during the Report Date Range (the last Program Name if there are several enrollments)
    • RRH Start Date - if the client has an RRH program enrollment (Project Type = 13) during the Report Date Range (the last Program Start Date if there are several enrollments)
    • Move-In - if the client has an RRH program enrollment (Project Type = 13) during the Report Date Range and the Head of the client’s household has a move-in date
    • Prevention Program Name - if the client has an HP program enrollment (Project Type = 12) during the Report Date Range (the last Program Name if there are several enrollments)
    • Prevention Start Date - if the client has an HP program enrollment (Project Type = 12) during the Report Date Range (the last Program Start Date if there are several enrollments)

Drilldown Functionality

Since this report exports as an Excel workbook, drilldown is not available.
